Why Buy Satanic?
Satanic is the item that turns a low-HP carry from a liability into an execution threat. The passive gives 30% lifesteal on every attack all game long. The active—Unholy Rage—cranks that lifesteal to 175% for six seconds and throws a basic dispel on top. That combination means one thing in practice: if you have enough attack damage and a target in front of you when Unholy Rage is active, you will go from 20% HP to full in three or four hits. Not almost full. FULL. The enemy team commits everything to kill you; you pop Satanic, and suddenly the hero they thought was dead is healing for more per attack than most heroes have HP regen per second. It is one of the most disgusting turnaround mechanics available to any carry in the game, and the basic dispel on activation removes slows, silences, and most debuffs—meaning you can also use it to escape a bad situation cleanly without any other dispel item in your inventory.
The passive 30% lifesteal is not a small number either. Against a target you are already winning against, it keeps you topped off through the entire fight without needing to activate the cooldown at all. You save Unholy Rage for the moment you are actually threatened and spend the rest of the fight healing passively through every right-click.
When to Buy Satanic?
The fourth or fifth item for most carries, after your core damage and survivability foundation is set. Satanic rewards attack damage—lifesteal heals based on the actual damage your attacks deal, so a carry already sitting on Daedalus, Desolator, or high base damage sees massive healing numbers per swing. On a hero with 150 damage, Satanic actively heals about 262 HP per hit. On a hero with 400 damage, that same active heals about 700 HP per hit. The gap is enormous, which is why you build damage first and Satanic later rather than rushing it early when your attack damage is still low.
Do NOT build it as your primary defensive answer against heavy burst magic lineups. Unholy Rage does not protect you from magic damage—it only heals from PHYSICAL attack damage. A Lina, Zeus, and Skywrath lineup that nukes you from 100% to 0% in 1.5 seconds before you can attack anyone is not solved by Satanic. That is a BKB problem. Satanic solves prolonged physical fights and sustained damage situations, not instant magical kill combos.
Tips & common mistakes
- · Activate Unholy Rage at LOW HP, not full HP. The entire point is that it heals you back to full from a dangerous state. Players who panic-pop it at 60% HP when they are not yet in danger burn the 30-second cooldown for nothing. Wait until you are below 30% and THEN pop—two or three attacks at 175% lifesteal on a high-damage target bring you back to fighting health instantly.
- · The basic dispel activates on CAST before the lifesteal boost begins. This means you can use Satanic to remove an Orchid silence, a Skywrath slow, a Nova root—even if those debuffs would prevent you from attacking, the dispel fires first and clears them. Most players don't know this and waste the item by trying to attack while silenced instead of popping Satanic to break the silence first.
- · Satanic does NOT work on magic or pure damage. It only heals from physical attack damage. If you use Unholy Rage while a witch doctor is death-warding you, the ward's damage does not trigger lifesteal. You are still dying. Get out of the Ward. · Missing attacks gives you zero lifesteal. If the enemy has evasion—Butterfly, Windranger's Windrun, or any hero with innate dodge—your Unholy Rage window is wasted on misses. Build Monkey King Bar before Satanic when evasion heroes are in the enemy lineup.
- · Satanic disassembles—the Morbid Mask component can be pulled out and used for other builds if your game plan changes. Keep this in mind when planning your item progression, especially if you started with Mask of Madness earlier and already have the Morbid Mask sitting in your inventory.
Summary
Satanic is a late-game monster item that rewards patient, damage-heavy carries who know when to activate it. Buy it after your damaged foundation is set, pop Unholy Rage only when you are actually low and in danger, and understand that it solves sustained physical fights—not instant magical bursts. When the conditions are right and you hit Unholy Rage at 15% HP into a clustered enemy team, there is no item in the game that produces a more dramatic turnaround than this one.
